General Description
Provide each of your assigned residents with routine daily nursing care and services in accordance with the resident's assessment and care plan, and as may be directed by your supervisors.
Essential Duties
- Adheres to the policies, operating procedures, the Mission and Core Values of Omega Senior Living.
- Responsible for completing the daily shift report and communicating directly to the next shift any urgent needs.
- Perform all assigned tasks in accordance with our established policies and procedures, and as instructed by your supervisors. Demonstrates ability to manage assigned tasks within the scheduled shift.
- Assists with resident care to include housekeeping, laundry, social needs, and demonstrates proper lifting techniques.
- Assists with serving meals and essential clean-up.
- Record all entries on flow sheets, notes, charts, etc., in an informative and descriptive manner.
- Demonstrates skills in meeting the physical and emotional needs of the resident.
- Report all changes in the resident's condition to the DON/Charge Nurse/HWD as soon as practical.
- Report all accidents and incidents you observe on the shift that they occur.
- Agree not to disclose resident's protected health information and promptly report suspected or known violations of such disclosure to the Administrator.
- Assist the DON/HWD with implementing any required plans of corrections following surveys or audits.
- Will keep certification current and participate in continuing education to include staff meetings, in-services, and web-based trainings.

Experience
- Must possess, as a minimum, a high school education or its equivalent.
- Prefer prior experience in Assisted Living. Prefer prior experience as a Care Partner or similarly situated position for at least 1 year.

Certified Medication Aide - FT/PRN

General Description
Provides administering of medications to residents as ordered by the attending physician, under the direction of the attending physician, the nurse supervisor or charge nurse, and the Director of Nursing Services. The administration of medications shall be in accordance with established nursing standards, the policies, procedures, and practices of this facility, and the requirements of this state. Provides direct care to residents following the individualized care plan.
Essential Duties
- Adheres to the policies, operating procedures, the Mission and Core Values of Omega Senior Living.
- Adheres to the policies and operating procedures of the community governing the administering of medications to residents.
- Exercises independent judgment and discretion in the performance of personnel tasks with the understanding that the responsibilities are not routine or limited to resident care matters. Maintains a clean, safe environment in the community.
- As per state regulations and completion of required training/certification, administers or assists with self-administration of medication and treatments as prescribed by the health care provider. Observes resident's response to existing treatment plan, is aware of prominent medication interactions and side effects, and observes resident for occurrence of such and notifies supervisor. Adheres to community infection control policies and procedures.
- Responsible for daily documentation in resident records according to state regulations where applicable, such as completing necessary forms, charts, reports, etc., and submit these as may be required. Observes and documents the health and emotional condition of each resident, promptly reporting all changes to the supervisor.
- Follows the facility's procedures allowing residents to self-administer his or her medications. Monitoring and documenting all aspects of such self-administering. Exhibits understanding of and follows medication policies and procedures.
- Maintains an organized and clean medicine supply room. Completes an accurate accounting of all medications, including controlled substances as per policy. Assists with completion of medication cart audits per policy and direction of supervisor.
- Receives report on all residents.
- Provides direct resident care.
- Follows procedure regarding incidents/accidents to residents.
- Continuing education to include staff meetings, in-services, and web-based training.
